Three 18-hole rounds of golf with tee times
beginning at 8 a.m. If you are playing the 2-Person Best Ball Team Event only, play is Friday & Saturday. Sunday will be the final round for those entering the Stroke Play Event.

Please complete an
entry form and mail it WITH THE ENTRY FEE before Friday,
June 13, 2014. Entries postmarked after June 13 will be accepted on a space-available basis (up to 80 golfers). No
entries will be accepted after Friday, June 20st . You may enter as a single and
participate in the Stroke Play Tournament OR bring a partner and enter the Best
Ball Event and the Stroke Play Tournament. Entry in the Best Ball Event
automatically enters you in the individual stroke play tournament. Both events
are held simultaneously on the tournament dates. Entry forms are available at your local club or at website: www.lwmga.org.
